How The Practice Uses Personal Health Information
The law sets out how we can use your personal health information. The Data Protection Act 1998 gives you rights about how your personal information is used, including the right for you to see the information we hold about you.

To give you the care you need we keep information about you including any illnesses, tests, prescriptions and treatment you have received. Access to this information is limited to staff involved in your care and treatment. This personal health information is stored securely on computer or on paper, or both.

We sometimes share relevant information with other organisations involved in your healthcare. For example, when your GP refers you to a specialist at the hospital we send relevant details about you in the referral letter. We also need to use your personal health information for administration tasks, but we only use relevant information. No information is passed to any non-clinical organisation outside of the surgery without your written consent.

From time to time the quality of care we provide is audited by other GPs from outside the area. If you would prefer your records not to be audied in this way please inform Mike Salt our practice manager.

If you have any questions about how we use your personal health information please contact Mike Salt. If you would like to see your health record there is a form that you will need to sign - these are available at reception - and there will be a £10.00 access charge. A further charge may be levied if any copies or printouts are requested. Prior notice will be required and a mutually convenient time will be arranged.

A practice information sheet on these issues is available from the surgery or for a download click Data Protection.pdf.
