How to join the practice.


If you intend to live in the practice area for more than 3 months you may apply to register as an NHS patient.

 

All newly registering patients will be asked to complete a questionnaire and to make an appointment for a health check with our nurses. If you are in the area for less than three months you may be able to receive treatment on a temporary basis. Ask at reception for details.

 

If you wish to register with our practice please ask for a registration form for each person who wishes to register. The form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor.

 

If you have your medical card available please attach it to the form. This information helps the Health Authority to quickly trace your medical records and send them to your new doctor. Please note that everyone over sixteen years will need a new patient check - a basic medical check - with one of the Nurses before they are fully registered.


If you would like to use our on-line appointment booking and prescription service, EMIS ACCESS, please let the receptionists know when you register.
